Skilled Nursing

Glenmeadow's Director of Resident Services is a licensed social worker who will work with you and your family to determine if a transition to a skilled nursing environment is appropriate. The majority of residents live out their life at Glenmeadow. In most cases, one-on-one care can be increased as needed in your existing apartment. If a move to a skilled nursing home is needed, however, we will work with you to identify the facility that best serves your requirements and will help coordinate the admission process.
